These terms and conditions accompany the Work Order Authorization. Customer is deemed to accept these Terms and Conditions by submission of this form or by commencement of work. Acceptance supersedes any other conditions, representations, or understandings with respect to this Work Order. JetAvenue LLC reserves the right to halt work if authorization cannot be confirmed.
